Adam doesn't always nurse until he falls asleep anymore, before his heart surgery he almost always fell asleep nursing, but these days he only does so if he's sleepy. Often times he nurses then talks to the boob for a while before trying to sit up.
This time he was tired and was rubbing both eyes while nursing.
